    Yes.

    All financial transactions over $10,000 have to be reported to the goverment, and they need it to file the report.

    Yes.

    They need your SSN for several reasons. One of which is to report the claim to the IRS, as they are required to do by law.

    You have to if you want to get paid – They have to report the payout to the IRS and require your SSN as the payee.

    If you don’t, you won’t get any settlement.

    They need that so the IRS can tell if you are trying to claim medical deductions that you were reimbursed for. You can only claim deductions for medical costs that were not reimbursed or paid by someone else.
